I came into the shelter as a owner surrender on 7/12/2019, with the surrender reason stated as person circumstance- loss of income.

Hazel is at risk for behavioral reasons. Hazel has displayed fearful behavior at the care center, and becomes nervous during handling. Hazel would be best suited for placement with a new hope partner that can provide the necessary behavior modification. Medically, Hazel seems healthy.

Hi! I'm Hazel! I'm named after a small tree which is appropriate since I'm so petite. And like the tree, I will grow and become strong. I may be a wilted flower right now but my life has changed and things are scary. I think things will get better once my new family finds me. Once I set up roots, I can be friendly and sweet. [DVM Intake] DVM Intake Exam Estimated age: 4 years Microchip noted on Intake? No History : Owner surrender. Subjective: BAR Observed Behavior -Leash reactive when we tried to get her out of kennel. She was growling and lunging towards leash. Had to give her oral sedation in a meatball to get her out. For exam she was tense with tail tucked but she allowed handling. During exam she did not lunge, growl, bark or try to bite. Evidence of Cruelty seen -No Evidence of Trauma seen -No Objective T = P =120 bpm R =eup BCS 5/9 EENT: Eyes clear, mild wax AU, no nasal or ocular discharge noted Oral Exam: Limited exam, muzzled, nsf PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: F/I, spotting blood, enlarged vulva, appears to be in heat MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Clean externally Assessment: -In heat -Otherwise apparently healthy Prognosis: Good Plan: -Sedation: 50 mg Acepromazine PO one hour before exam -Cleaned ears AU -Trazadone 150 mg PO BID until otherwise directed since she is anxious/very nervous -Schedule surgery asap SURGERY: Okay for surgery

Anesthesia Summary: Canine Spay Pre Medication: Hydromorphone 2mg/ml - 0.20 ml IM, once Dexmedetomidine 0.5 mg/ml – 0.32 ml IM, once. Induction: Propofol 10 mg/ml – 7.00 ml IV, once. Anesthesia Notes: Size 10 fr. ET tube placed, maintained general anesthesia throughout procedure on variable isoflurane and variable O2. Used (rebreathing) system with 3 L bag. 20 g IVC placed in R cephalic vessel. Intraoperative IV LRS at 5-10mL/kg/hr at 200 mL/hr NSAID: Rimadyl 50mg/mL injectable, 1.60 mL, SQ, once post-operatively, for post-operative pain relief. Propofol given 0.7 ml IV @ 12 pm during sx. 1 ml @ 12:05 pm Recovery Status Uneventful Anesthetist/Surgical Monitor 16/1390 Start 1 tablet of Carprofen 100 mg SID PO x 4 days as pain management. --In white box—

Basic Information:: Hazel is a large mixed female breed approximately 4 years old who was surrendered due to unforeseen circumstances

Previously lived with:: 2 adult

How is this dog around strangers?: Around strangers Hazel will smell you at first, is shy for a few days but once she warms up she is very friendly and sweet.

How is this dog around children?: Hazel has previous been socialized with children ages infant to 4 years who Hazel is great with. Hazel is friendly,affectionate and playful.

How is this dog around other dogs?: Around large mixed breeds Hazel will walk with them and is super friendly nut with small mixed breeds which is will growl, bark and lunge towards them.

How is this dog around cats?: Previous owner did not have any cats in the home so behavior not observed

Resource guarding:: Hazel isn't bothered when her food or treats are taken away but with her toys Hazel will try to bite the toy and accidentally nip.

Bite history:: No known bite history during time of intake

Housetrained:: Yes

Energy level/descriptors:: High

Other Notes:: Hazel isn't bothered when being held or restrained while on her harness. Hazel sleeps on the furniture with the owner so behavior unknown for how she would react if ever pushed off. Hazel loves getting baths and loves to be brushed. Hazel will sometimes bark when unfamiliar people approach her home or while on a walk but other times she will sniff the person for awhile.

Has this dog ever had any medical issues?: No

Medical Notes: Owner stated Hazel has a Yeast infection in both ears but did not provide any proof of vet documents

For a New Family to Know: Hazel is described as friendly, affectionate and playful with owner but around new people she is shy for a few days. Hazel loves playing with tennis balls, rope and tire toys. Hazel is indoors only. Hazel was fed dry food three times a day. Hazel is house trained and will rarely have accidents. Hazel will whine and bark when left alone at first but once she calms down she will take a nap. Hazel knows hand commands but not verbal. Hazel will tend to pull on leash. Hazel has been off leash while on a walk at night and she will walk with the owner.

Summary:: Hazel is fearful at the care center and growls and bites the leash when attempts are made to remove her from her kennel. She is unable to be removed from her kennel or receive a handling assessment at this time. Please see her owner surrender notes for a full report of her behavior in her previous home.

Summary:: Due to difficulty removing from kennel and not allowing handling, Hazel was not able to be introduced to other dogs in the care center. The previous owner of Hazel cited that she has interacted successfully with larger dogs, but will growl, bark and lunge at smaller dogs.

ENERGY LEVEL:: Hazel is described as having a high level of activity. We recommend long-lasting chews, food puzzles, and hide-and-seek games, in additional to physical exercise, to positively direct her energy and enthusiasm.

IN SHELTER OBSERVATIONS:: 7/16: Hazel has been extremely fearful at the care center and is unable to be removed from her kennel. While she will come forward and take high value treats at times, as soon as the leash is introduced she runs to the back of the kennel, growling and snapping at the leash. When attempts are made to leash her, she bites the leash.

Recommendations comments:: No children: Due to how fearful Hazel is currently at the care center, we recommend an adult only home. Place with a New Hope partner: Hazel is very fearful at the care center and does not allow a leash to be placed over her head. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement-based) and re-evaluate behavior in a stable home environment before placement into a permanent home.
