I came into the shelter as a aco impound on 2/10/2018.

Chip is at risk for medical reasons, having been diagnosed with an Upper Respiratory Infection, which is contagious to other cats. Behaviorally, we have no concerns for Chip, whom we believe would be suitable to most homes.

R = wnl BCS 4/9 - ravenous appetite EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: tachycardic, gallop arrhythmia, CRT < 2; Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: externally normal Assessment: poss. arrhythmia, underweight Prognosis: good - open Plan: rec' recheck heart rate/rhythm +/- gen BW if indicated SURGERY: Okay for surgery, recheck heart

Brief recheck Day 1 vomiting, and eating vomit, reported on Monitoring Board on 2/11 during Rounds. p is BAR, no C/S/D, no URI signs, no vomit or nausea noticed in cage during exam, mm pink and moist, euhydrated. A: vomit one time today P: CTM if vomiting continues. If so, consider abdominal rads vs cerenia trial, and SQ LRS

Tachycardia and intermittent gallop rhythm noted on intake; recheck to okay for sx S/O -QAR, docile and friendly, sweet boy! -good appetite -no nasal discharge or sneezing noted or reported -eupnic, clear lung sounds -no murmurs or arrhythmias noted, HR 120-140 -soft abdomen A 1. Previous gallop arrhythmia/tachycardia-suspect stress related on intake, appears resolved P -okay to neuter

S/O -QAR, docile, good boy but seems slightly lethargic -good appetite -persistent soft sneezing, excessive serous nasal discharge -mm pk, sl tacky; mild hypersalivation -eupnic -neuter site healing well with no scrotal swelling or irritation A 1. URI P -move to isolation -submit resp PCR swab to Idexx -150 ml SQF SID x 3 days -cerenia 0.4 ml SQ SID x 2 days -suspect viral URI so will not add antibiotics unless PCR positive for mycoplasma, mucoid nasal discharge develops or course is noted to be prolonged/severe

Diagnosed with URI but was suspected to be viral so no antibiotics added yet, resp PCR panel pending S/O -QAR -good appetite -mucoid nasal discharge, nasopharyngeal congestion -OU: mild blepharospasms -eupnic A 1. URI, possibly bacterial P -add doxycycline 0.8 ml PO SID x 10 days

KNOWN HISTORY:: Chip was brought in as a stray, so we don't have any behavioral history or tendencies in a home environment.

MEDICAL BEHAVIOR:: 2/10/18 Observed Behavior - excited/ravenous appetite, allowed handling, but resists restraint

ENRICHMENT NOTES:: Resting on bedding in main compartment, soft eyes and body. Stood and came to the front on approach, started meowing and rubbing against the bars. Leaned and arched enthusiastically into pets, started to roll over. Ate offered treats. Ideal event cat.
