Subjective: QAR, ptyalism Observed Behavior - came right out of carrier, no hissing/swatting/growling Evidence of Cruelty seen - N Evidence of Trauma seen - N Objective T = DNP P = 190 R = 24 BCS 5/9 EENT: Eyes clear, moderate ceruminous debris AU, mild serous nasal discharge, no ocular discharge noted Oral Exam: significant ptyalism, no foreign material visible in oral cavity, clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered male MSI: 1# wt loss since adoption 4 days ago; ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Externally normal Assessment: suspect URI based on signs noted by O - Convenia/cerenia/SQF given today (3/10/19) at Animal Speciality Center 1) Ptyalism 2) 1# wt loss since adoption 4 days ago 3) O reported coughing/sneezing/open mouth breathing but not noted during exam - mild serous discharge Prognosis: good w/ treatment Plan: monitor in medical iso, consider starting on doxycycline if signs not improving by tomorrow SURGERY: Permanent waiver due to already neutered.

S/O: Pt is alert in kennel, drooling and sneezing. No nasal or ocular discharge. E-tube bandage is clean and in place. A: URI - suspect oral ulceration P: 1. Simbadol 2. Magic mouthwash 1088

S/O - et tube placed previously due to anorexia - prolonged skin tent - marked pytalism - open mouth breathing - lethargy - not eating voluntarily EENT - purulent nasal discharge; eyes clear H/L - clear/eupnic ; nsr no arrhythmia A - URI - severe - dehydration, purulent discharge, anorexia P - Place on IV fluids @ 12 ml for 24 hours - Cerenia 0.35 ml IV SID for 3 days - Doxycycline 10 mg/kg PO for 10 days - Nebulize x 5 days

SO Cat with severe URI and anorexia. Feeding tube placed on 3/11. P continues to remain anorexic; is handling e-tube feedings well. QAR in kennel, easily roused EN -- eyes and nose are clear. no audible congestion. Oral -- ptyalism, intermittent gagging A URI, severe anorexia P mirataz topical 1.5inch q24h x 5 days Recommend P get immediate placement; P should be transferred to a hospital for continued care and monitoring.

SO Cat with severe URI and continued anorexia. A feeding tube was placed on 3/11. BAR in kennel, easily roused. Today, P shows interest in food by sniffing but gags immediately after. Did not eat on his own. EN -- eyes are clear. crusted mucoid nasal discharge on nares. Mild to moderate audible congestion and sneezing. A URI, severe anorexia P continue on current tx plan little remedies q12h x 3 days Recommend P get immediate placement; P should be transferred to a hospital for continued care and monitoring.

Lungs - clear eupnic MSKI - ambulatory x 4 CNS - normal mentation A - Moderate URI - Mild lethargy - Underweight - Still accepting esophageal feedings QID P - Continue supportive care - Change esophageal bandage NEW HOPE - Await NH placement - Will need an immediate follow up examination to monitor progress of URI
