Alessa is on the at risk list for medical reasons - Alessa is a very sweet old dog with multiple medical issues. She has chronic otitis and will likely require surgery on one or both ears. She also has multiple masses, most significantly mammary gland tumors. She should also have surgery to remove and biopsy those. Additionally, Alessa has two dermal masses, suspected osteoarthritis, and recently was diagnosed with CIRDC. Her CIRDC may be progressing to pneumonia because today she is lethargic and not eating. Behaviorally, Alessa has allowed all medical handling and is such a good girl.

Meet Alessa! Alessa came to us as a stray with unknown history after being found with another dog. From the moment she arrived, she has shown herself to be a gentle, friendly, and easygoing girl. She walks nicely on leash, explores her surroundings with a soft, relaxed body, and readily approaches people for affection. Alessa enjoys attention, wags her tail sweetly, and allows handling with ease - she's a calm companion who simply enjoys being near her humans. My Ideal Home Alessa would do well in a calm, comfortable home where she can enjoy a relaxed lifestyle. She has a loose leash walk and a soft demeanor, making her a great match for adopters looking for a gentle, steady companion. Given her medical needs, she will require a committed adopter prepared to provide ongoing veterinary care and follow-up. A quiet home where she can rest, receive regular medical support, and soak up affection would help her thrive. Good with Kids? Yes. Alessa has shown a soft, tolerant temperament during handling and does not display reactive behaviors. As with all dogs, respectful interactions are recommended. DVM Intake Exam Estimated age: geriatric - ~10yo based on ocular changes and graying fur Microchip noted on Intake? negative History: eviction hold Subjective: BARH Observed Behavior - friendly, tail wagging, soft bodied, seeks attention, able to examine with minimal restraint Is there evidence of Cruelty? n Is there evidence of Neglect? n Is there evidence of Trauma? n Objective BCS 5/9 EENT: Eyes clear, nuclear sclerosis OU, ears moderate purulen discharge AU, stenotic ear canals and crusting, no nasal or ocular discharge noted Oral Exam: No obvious lesions, staining and yellowing throughout PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Female suspect intact, no scar. Multiple firm mammary masses on caudal nipples, irregular, lumpy-bumpy and of various sizes. MSI: Ambulatory x 4, skin free of parasites, Multifocal round hairless dermal masses ~1cm each (one on left thorax, one on right carpus), healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Ext normal Wood's Lamp Exam: NP Assessment Geriatric medium mixed breed Multiple mammary masses, r/o neoplastic Multiple skin lesions Chronic otitis with stenotic ear canals Prognosis: Guarded Plan: Routine intake procedures Send out cbc/chem/t4 Clean ears and apply claro AU If considering placing, recommend 3 view chest rads. Placement may consider radical mastectomy and send out to pathology for grading. Ear canals are narrow and suspect has had history of ear infections. Likely end stage aural disease that will need to be surgically addressed if no improvement with medical management. Can consider sedated ear flushing. SURGERY: Permanent waiver due to age

It is the policy of ACC not to perform surgery on any animal over the age of 8-10 years due to the higher risks incurred in a shelter setting. The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

Recheck geriatric dog with comorbidities S/O: BAR, soft body, sweet girl, allows all handling; good appetite, sniffling, no c/s/v/d noted EENT: Mild mucoserous nasal discharge, no ocular discharge, nuclear sclerosis OU; ears - chronic otitis with stenosis and hardened canals palpable AD>AS ORAL: mm pink and moist, CRT <2; staining and wear throughout teeth with some mild crown fractures LUNGS: Eupneic MSI: Ambulatory x 4 with stilted/stiff gait; 2cm spherical hairless nodule at left thorax, 1cm round nodule at dorsal right front paw; multilobular mammary gland tumor - some parts palpate firm, some parts cystic, and one section looks like a skin tag CNS: Appropriate mentation A: CIRDC Geriatric Mammary gland tumors - irreglar, cystic, multilobular Dermal mass at left thorax Dermal mass at RF paw Chronic otitis AD>AS Dental wear/staining Suspect OA P: Start doxycycline 10 mg/kg PO SID x 10 days Recheck CIRDC day 10 Attempted to clean ears, but canals severely stenotic and unable to administer cleaning solution Seek placement for surgical consult for TECA and MGT removal CTM closely while at QACC

Noted to be acting lethargic on 2/23, had been dx with CIRDC and started on doxycycline on 2/21. Received supportive care 2/23. Placement had requested CXR S/o: Patient is BAR as she walks towards medical, seeking attention from handlers. EENT: Mild serous nasal discharge, no ocular discharge. H/L: No murmurs or arrhythmias, light wheeze on ventral lungs, no crackles. Normal RR and RE (~24BPM) MSI: Ambulatory x4, stiff gait; 2cm round nodule on left thorax, 1cm round nodule on dorsal RF paw, multilobular mammary gland tumor associated with caudal mammary glands. Neuro: Appropriate mentation A. CIRDC - apparent improvement from noted clinical signs yesterday Geriatric Mammary gland tumor Dermal mass L thorax Dermal mass RF paw Chronic/end stage otitis Dental wear Suspect OA P. Sedated with 0.3ml dex/0.3ml torb 3 view chest rads: Possible increased opacity over hilar region, subjective R sided heart enlargement. No apparent masses/mets or free fluid. Reversed with 0.3ml antesedan IM - recovered uneventfully Continue doxycycline for ~10 days +/- supportive care Recommend placement to follow up with rDVM to consider surgical removal of masses.

Summary:: Due to Alessa entering the facility as a stray, there is no known history of interactions with other dogs prior to intake. 02/20/26: Alessa was introduced to a novel male dog. At the gate, Alessa approached with full-body hackles, displaying a nervous and cautious posture. Following mutual greetings, she disengaged and walked away, engaging in displacement scenting, concluding the interaction.

Summary (7):: 2/20/26: Alessa is laying down in her crate as handler approaches. She is able to be slowly leashed and she is slowly taken to the play yard for play group. In the yard, she greets the other dog at the gate with a tense body and her hackles raised due to the other dog constantly barking at her. Please see dog to dog notes for more details. She is able to slowly return back to her crate with no issues. 2/13/26: Alessa is standing in her crate as handler approaches. She is able to be leashed with ease and is then taken to the behavior room for her assessment. In the room, she has a no interest interacting with the handlers. She would explore around the room with sniffing. She has a tense body and would lip lick while being petted. Please see behavior assessment notes for more details. She is able to return back to her crate with no issues.
