DVM Intake Exam History: ACO impound, litter of 6; littermates with ringworm lesions (woods lamp positive) Estimated age: Approx 3 weeks Microchip noted on Intake? [Spay/Neuter Waiver - Temporary] Your newly adopted pet has been diagnosed with being underweight and the staff veterinarians are issuing a TEMPORARY waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment through to the resolution of the issue. At the time of a full recovery you may choose to have your veterinarian perform the spay/neuter surgery, or make provisions to return the pet to ACC for sterilization.

Weight: 0.42kg Respiratory- Mild cough/wheeze, crackles on left lung fields. Chest Rad- Difficult to fully access due to patient size. No obvious consolidation. Treat as pneumonia while crackles present. Assessment: Pneumopathy- Suspect Pneumonia - r/o aspiration vs infectious vs other. Historic: Conjunctivitis - Infectious - TR Dermatophytosis (ringworm) - TR Neonate < 4 Weeks Old - Orphaned - TM Ocular abnormality-unspecified - TM Upper Respiratory Infection – Mild, Feline - TR Plan: Clavamox Drops- 13.75mg/kg PO BID - 0.1ml PO BID x 10d Recheck Daily

recheck kitten, possible pneumonia noted yesterday S/O QAR, no c/s/v/d, reportedly eating well, weight gained, 472grams today EENT: no ocular or nasal dc, mm pm <2 HL: nma, clear, eupneic MSI: amb x 4, good skin turgor A. Ringworm- under tx Pneumonia - not appreciated today, no URI signs but quiet P. Con't with amoxi-clav drops for now- recheck tomorrow. If no pneumonia/URI signs appreciated tomorrow, consider d/c'ing

Recheck URI/ringworm kitten, gaining weight; concern for pneumonia a couple days ago and started clavamox S/O: QAR-BAR with handling, eats kitten food with a lot of encouragement, log indicates eating syringe feedings well, no c/s/v/d noted EENT: Eyes sealed shut with mucoid discharge, mild serous nasal discharge LUNGS: Eupneic CNS: Appropriate mentation A: Weaning kitten URI, conjunctivitis Ringworm P: Cleaned eyes Restart olfoxacin OU BID Continue clavamox, lyme dips, itraconazole CTM closely on rounds
