DVM Intake Exam Estimated age: >=10 years Microchip noted on Intake? Negative History: Stray Subjective: BAR MM pink, moist, CRT <2 sec Observed Behavior - Blue; Able to examine, nervous, body tense, vocalizing Is there evidence of suspected cruelty? - No Objective: P = 180 hr R = 36 rr (vocalizing) BCS 7/9 Wt = 12.1 lbs EENT: Lenticular sclerosis. Ears contain moderate amount of red-brown sandy, coffee ground-like debris and with positive pinna-pedal reflex. Minor abrasion with scabbing on right anterior cheek. Oral Exam: Severe dental disease with calculus, gingivitis, and multiple missing teeth; incomplete exam. PLN: No enlargements noted H/L: NSR, no apparent murmur, CRT < 2, Lungs clear, eupneic ABD: Mild-moderate cranial to mid-abdominal distension with ventral tautness. U/G: MN (green neuter tattoo observed) MSI: Ambulatory x 4. 3-4 inch fluctuant, soft, multi-lobulated subcutaneous mass on right ventrolateral neck/cheek with overlying alopecia. CNS: Mentation appropriate - no signs of neurologic abnormalities Assessment: Geriatric with suspect chronic conditions -Otitis externa: Suspect *Otodectes cynotis* infestation -Dental disease: Severe periodontitis -Subcutaneous mass: Right ventrolateral neck/cheek; R/O abscess, cyst, neoplasia -Abdominal distension: R/O organomegaly (hepatomegaly, splenomegaly) vs other -Minor abrasion, right cheek -Overweight Prognosis: Guarded Plan: Standard Intake Procedures for Cats Performed -Blood collected for adult wellness panel (send-out); results pending -Ear cleaning performed -Selamectin applied topically; repeat in 1 month -Recheck in 1 day for bloodwork results -Recommendations pending bloodwork: sedated exam for complete oral evaluation, mass evaluation (FNA), and potential abscess care. Recommend thoracic and abdominal radiographs. -Clavamox 62.5 mg tablets (11.4 mg/kg). Give 1 tablet PO BID x 7 days. SURGERY: Permanent waiver d/t age

Recheck, bw results S/O: BAR, good appetite, no c/s/v/d noted EENT: Mild dark discharge AU, no ocular or nasal discharge LUNGS: Eupneic MSI: Amb x 4, large fluctuant swelling at right ventrolateral neck, healed wounds on swollen area, one small scab at right side of face adjacent to swelling CNS: Appropriate mentation CBC: eosinophils 2.034 (H) CHEM: chloride 112 (L), total protein 10.0 (H), globulin 6.6 (H) T4: wnl *Sedated with dex/torb 0.12 ml IM of each. -Clipped and scrubbed swelling, then lanced with #10 blade. Copious amount of serous fluid expressed out. -Flushed with sterile saline, no additional discharge/fluid A: Swelling at ventrolateral neck - ro cyst vs abscess vs other Senior cat Dental disease Ear mites P: Onsior 2 mg/kg SQ once Ivermectin 0.16 ml SQ once Cleaned ears again Full atipamezole reversal, smooth recovery No further treatment Recheck swelling in 5 days CTM while at QACC

Brief recheck exam S/O: BAR, good appetite, food dishes empty, no c/s/v/d noted EENT: No ocular or nasal discharge LUNGS: Eupneic MSI: Amb x 4, large fluctuant swelling at right ventrolateral neck, healed wounds on swollen area, one small scab at right side of face adjacent to swelling CNS: Appropriate mentation A: Swelling at ventrolateral neck - recurring 6 days post lance / drain / flush r/o cyst vs sialocele vs underlying mass Senior cat Dental disease Ear mites P: CTM while in care, consider analgesics if swelling becomes painful Consider ARL - P may need additional diagnostics and possible repeat surgery and if swelling continues to grow could compress cervical structures

Recheck, cystic mass on neck refilled quickly after lancing S/O: BAR, active, leans into petting, good appetite, no c/s/v/d noted; no ocular or nasal discharge, eupneic; large fluctuant mass at right side of neck with a couple more firm nodules palpable adjacent. A/P: Senior cat, cystic mass, dental disease; recommend mass removal +/- biopsy with placement; CTM while at QACC.
