I came into the shelter as a owner surrender on 9/5/2026, with the surrender reason stated as person circumstance - hoarding situation.

DVM Intake Exam Estimated age: 1 year based on the condition of teeth and eyes Microchip noted on Intake? Negative History: Owner surrender - hoarding, under socialized, no name, no care, etc. Subjective: BAR H pink 1 sec Observed Behavior - Hiss, swat, lunge. He thinks he's fighting for his life. Evidence of Suspected Cruelty seen - No Objective P = 80hr R = 40rr BCS 6/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted, debris in ears but no mites seen on slide. Oral Exam: NSF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI testes x 2 MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Assessment Apparently healthy Prognosis: Good Plan: Sedate with 0.54 mLs DKT premix dexdomitor (0.0125 mg/kg), telazol (0.25 mg/kg), ketamine 5 mg/kg). Hx: URI, decreased appetite S: QAR and tense inside of den. Dry food appears disturbed. Did not eat gabapentin or meds bowl. POTENTIAL CHALLENGES:: Fearful,New home adjustment period

BEHAVIOR DETERMINATION: : New Hope Only

BEHAVIOR SUMMARY:: Waffle Fry appeared highly fearful and anxious during the assessment, remaining tightly crouched inside the den upon approach. When the den cover was lifted, he quickly lunged toward the plexiglass door and hissed, indicating a strong defensive response to the perceived threat. He avoided contact and anxiously retreated, showing that he was not comfortable with interaction at that time. The assessment was ended to help minimize his stress and allow him to decompress. Waffle Fry is displaying behaviors that preclude placement in the adoptions room and/or may require further investigation before placement in a home. He is extremely fearful in the shelter environment and does not currently tolerate petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time.
