Ears clean, no nasal or ocular discharge noted Oral Exam: marked generalized tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, full cranial abdomen - possible mass or organomegaly U/G: male neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry unthrifty hair coat with moderate dandruff, moderate dorsal muscle atrophy CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: externally clean Wood's Lamp Exam: Assessment: Senior/ Geriatric MN DSH -Dental dz - grade 4/4 -Moderate sarcopenia -Cloudy cornea with mass in anterior chamber - r/o uveal cyst vs inflammation vs neoplasia, r/o glaucoma -Possible abd mass or cranial abd organomegaly Prognosis: Fair to guarded Plan: Routine intake tasks. Fluorescein stain - no corneal ulcers OU CBC/ Chem/ T4 submitted Placed nosorb to get urine for add on UA LVT to get BP in AM Monitor while in ACC. Recheck eye and overall once BW returns. SURGERY: Already neutered

Progress exam S: BARH - friendly, soliciting attention, leans into cheek scratches pale pink moist mm, CRT <2s O: EENT: OD - ~1cm collection of purulent debris in ventral aspect of anterior chamber, aqueous humor translucent - no blepharospasm, no discharge (appears comfortable) OS WNL Ears clean, no nasal or ocular discharge noted Oral Exam: marked generalized tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Large cranial mass palpated unable to palpate origin U/G: male neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry unthrifty hair coat with moderate dandruff, moderate dorsal muscle atrophy CNS: Mentation appropriate - no signs of neurologic abnormalities A: OD hypopyon, uveitis with secondary glaucoma - underlying cause open Large cranial abdominal mass - suspect malignant neoplasia due to echogenic appearance, origin unclear (susp gastric or hepatic due to location but unable to confirm) HX borderline hypertension r/o white coat hypertension vs true HX severe dental disease Moderate sarcopenia Geriatric MN DSH Prognosis: Poor long term P: Fluorescein stain OU no uptake IOP (tonovet) OS 19mmHg, OD 23mmHg Susp OD IOP high end of normal due to concurrent uveitis and 2ary glaucoma AFAST - large heterogenous echogenicity / cavitated abnormal section of tissue in cranial abdomen - P ran out of kitty minutes, unable to scan through abdomen to confirm origin ADD Neopolydex OD BID x7d --> 4/19 Seek placement for possible ophthalmology / oncology consultations vs palliative care Consider repeat AUS another day, allow time to cool down CTM at QACC - P is stable at this time but if P declines consider EHR due to poor prognosis Brief progress exam 4/14
