N Objective: T = np P = wnl R = wnl BCS 5/9 EENT: eyes sealed shut with mucopurulent dc, copious mucopurulent dc seeps out when eyes soaked, severe conjunctival hyperemia and chemosis, only small area of cornea visualized OU, ears clean, no nasal discharge noted Oral Exam: deciduous inc, max canines erupted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Wood's Lamp Exam: negative Assessment: Neonate orphaned Severe conjunctivitis Prognosis: fair Plan: intake exam and tasks eyes soaks 10mL SQF once 0.1mL b12 SQ oflox 1-2 drops OU BID x 7 days set up incubator, feed per "yellow" kitten recheck eyes in 2 days awaiting sort SURGERY: Okay for surgery: N Temporary waiver due to size

Brief recheck conjunctivitis S: BARH - at front of kennel soliciting attention meowing pale pink moist mm at distance no c/s/v/d reported eating well from slurry syringe gaining weight O: EENT: no nasal discharge, OU mild ocular discharge and blepharospasm persist but subjectively improved from prior report (scant to no chemosis) LUNG: Eupneic at distance INT: Unkempt haircoat MSK: ambulatory x4 CNS: appropriate mentation A/P: Conjunctivitis - improving CWCP - DVM recheck at end of ofloxacin course

Recheck conjunctivitis, log indicates good appetite (eating on own) S/O: BAR, no c/s/v/d noted; kennel dirty with food and feces on bedding EENT: No ocular or nasal discharge LUNGS: Eupneic MSI: Amb x 4 CNS: Appropriate mentation A: Conjunctivitis - apparently resolved Weaned kitten P: Move to larger kennel with littermates CTM on rounds

Orphaned neonates No signs of FPV aTT No signs of conjunctivitis in littermates aTT P. CTM and recheck as scheduled
