Your newly adopted pet IS UNDERAGE and the staff veterinarians are issuing a TEMPORARY waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment through to the resolution of the issue. At the time of a full recovery you may choose to have your veterinarian perform the spay/neuter surgery, or make provisions to return the pet to ACC for sterilization.

Concern for dermatophytosis based on suspicious lesion on RF Subjective: BAR Observed Behavior - vocal, active, squirmy with restraint Is there evidence of suspected cruelty? no Objective: P = 200 R = 24 BCS 4/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, live fleas, no masses noted, scaly circular area of alopecia on RF at the level of the elbow (approx. 0.5 inch diameter) CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: externally normal Wood's Lamp Exam: lesion on RF is positive Assessment: Approx. 6wk MI DSH fleas woods lamp positive for dermatophytosis Prognosis: good Plan: intake tasks capstar PO start itraconazole approx. 5mg/kg PO SID x 30 days then recheck PCR in 21 days Re-weigh weekly and adjust dose as needed Submit dermatophyte PCR today - DVM follow up in 48 hours Move to ISO Seek placement asap SURGERY: Temporary waiver due to age/weight and dermatophytosis

Some kittens have lesions consistent with ringworm, PCR pending. PLAN: Adjusting treatment -Add lyme dips twice weekly -Adjust itraconazole dose to ~10 mg/kg PO SID (dosing based on lowest weight kitten) -Pend PCR results

DVM Lab Work Interpretation History: Ringworm-like lesions observed; PCR tested on 7/2/26. Test Result: Microsporum spp. and M. canis RealPCR Positive. Plan: -Already under full treatment for Ringworm since 7/2. --Lyme dips twice weekly 7/2-7/30. --Itraconazole ~10 mg/kg PO SID (dosing based on lowest weight kitten) 7/2-7/23. -ACRs now placed: --Weekly ringworm check (litter of 5) - 7/9 --Weekly ringworm check (litter of 5) - 7/16 --Pooled ringworm PCR (litter of 5) - 7/22 --Recheck ringworm (final?), PCR results?
