Hello, my name is Flare. My animal id is #253842. I am a male orange tabby cat at the Manhattan Animal Care Center. The shelter thinks I am about 3 years 6 months old.
I came into the shelter as a stray on 5/6/2026.
Flare has been placed on the at-risk list for medical concerns. Flare has feline lower urinary tract disease and has repeatedly obstructed despite urinary catheter placement. He needs to be transferred to a veterinary hospital for a perineal urethrostomy.

DVM Intake Exam Estimated age: 3 yr Microchip noted on Intake? negative History:stray Subjective:stray Observed Behavior - lunging hissing and growling at kennel, sedated for exam Is there evidence of suspected cruelty? none Objective: QAR mild dehydration mm pink P = 100 R = WNL BCS 5/9 EENT: Eyes crusted ocular discharge, ears clean, right pinna has wounds and swelling, crusts removed with small amount of blood no nasal or ocular discharge noted Oral Exam: clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: firm moderately sized bladder unexpressable, scrotal testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal:not performed Assessment: UO, non azotemic wound on ear slight anemia HCT 28.2% Prognosis:guarded Plan: sedation Dexdomitor 0.1ml Butorphanol 0.1ml ZOrbium green tube dose topical Onsior 2mg/kg SC Crenia 1mg/kg SC left cephalic 24 g cephalic IV catheter LRS fluid bolus 30ml bolus over 30 min, set at 15ml/hr urinary catheterization 3.5Fr passed with minimal difficulty, some grit encountered. Urine slightly cloudy, dark yellow , floating white pinpoint body in sample.Bladder was flushed with 50mls LRS through catheter UA out ( via free catch) CHEM/lytes BG 311 CBC HCT 28.2%, NEUT 5.07, PLT 0.65 recheck tomorrow SURGERY: Temporary waiver due to urinary blockage

SO: Urinary obstruction, urinary catheter placed yesterday. Was not azotemic BAR, cheek rubbing all over kennel door. Urinary catheter on floor of kennel. Eating s/o diet with ravenous appetite eent: eyes clear, no ocular or nasal dc apparent h/l: eupneic, no coughing or sneezing appreciated ug: bladder small and soft on palpation msi: ambulatory x4 neuro: mentation alert and appropriate A: UO post obstruction P: monitor for urine production

Has been seen straining in litter box intermittently throughout day BAR in kennel, offered urinary s/o canned diet and ate with ravenous appetite Allows for brief palpation of abdomen, bladder small, but does growl during palpation A: FLUTD P: urine production has not been significant, but bladder remains small and eating urinary diet with very good appetite continue to monitor for urine production

SO: FLUTD with obstruction, urinary catheter placed 5/7, but was removed by Flare overnight. 5/8 bladder remained small all day and was eating s/o with very good appetite, but was straining to pass urine. QAR, tense, but allows for brief palpation of bladder, some hematuric urine in bedding eent: eyes clear, no ocular or nasal dc apparent h/l: eupneic, no coughing or sneezing appreciated msi: ambulatory x4 ug: large firm bladder on palpation neuro: mentation alert and appropriate A: FLUTD with obstruction P: sedate for replacement of urinary catheter 0.1ml dexmedetomidine 500mcg/ml, 0.1ml ketamine 100mg/ml, 0.1ml butorphanol 10mg/ml IM

Issue List: - FLUTD with obstruction; has been removing urinary catheter repeatedly despite e-collar - eating some QAR DH eating; urine in litter box EENT: no nasal or ocular discharge noted H/L: eupneic PLN: WNL ABD: small soft bladder and easily expressed (urine bloof tinged) U/G: MI MSI: Ambulatory x 4, urine soiled hair coat CNS: Mentation quiet - no signs of neurologic abnormalities A) FLUTD with UO prognosis guarded P) CWSC Telazol- 0.16 ml IM urinary catheter removed by patient again; will see how the cat is urinating naturally (has removed urinary catheter repeatedly and there is urine in the litter box) if re-blocks again consider PU surgery recommend placement on ARL SQ fluids- 150 ml LRS medical bath

S: UO cat has been unblocked twice already, struvite crystals present on UA. Catheter removed yesterday. O: Found listless in cage today with a large, hard bladder and crystalline debris at tip of penis. Unable to express bladder. A: UO X 3 P: sedate with 0.54 mLs DKT (0.0125 mg/kg dexmedetomidine, 0.25 mg/kg telazol, 5 mg/kg ketamine) IM Passed a tomcat catheter with minimal difficulty. Blood clots and debris in urine. Did not reverse. Needs a PU Sx. Already on ARL.

BEHAVIOR DETERMINATION: : Level 3
BEHAVIOR SUMMARY:: Flare was tightly crouched on his bedding upon approach, alert with wide eyes and an intense stare. He allowed petting but shrank in place throughout the assessment. During pick up, he tensed up, and handling was ended to help reduce stress. Flare tolerates attention and petting but appears fearful or stressed in the shelter. He may be a little more independent, and may need time to warm up to his new home. We recommend that this cat go to a home with experienced cat parents.
