N Objective: T = np P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: moderate wear and staining to visible dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated, distended abdomen, no palpable masses U/G: MI, two scrotal testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, focal alopecia down dorsum, no overt lesions, diffuse muscle wasting especially over the head and epaxials CNS: Mentation appropriate - no signs of neurologic abnormalities Assessment: Alopecia Geriatric- suspected underlying condition dental dz Prognosis: fair pending BW results Plan: intake exam and tasks CBC/CHEM/t4 to lab, check for results tomorrow Dipstick (urinated on floor prior to exam) , appears to have USG ~ 1.020 awaiting sort SURGERY: Okay for surgery: N Permanent waiver due to age

It is the policy of ACC not to perform surgery on any animal over the age of 8-10 years due to the higher risks incurred in a shelter setting. The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

CBC - Mild neutrophilia 11.035K/uL - Mild monocytosis 1.073K/uL - Eosinopenia 0.015K/uL Chem - Mild elevated SDMA 17 - Mild elevated BUN 33 - Normal creat 1.2 - Mild hypochloremia 106 - Mild elevated ALP 176 T4 0.8 (low) USG at intake 1.020 Exam due to flagged hindlimb lameness BAR, initially whale eye when attempting exam but then allowed most handling with food distraction, eating treats readily EENT: No ocular or nasal discharge H/L: Eupneic ABD: Mild-moderate abdominal distention MSI: Amb x4, moderate hip sway, moderate bilateral hindlimb lameness with moderate circumduction and mild dragging/knuckling over. Limited ortho exam tolerated - possible decreased ROM of hips bilaterally, no overt crepitus. General hindlimb weakness appreciable - standing hindlimb collapses without support when one hindlimb lifted. Alopecia in a symmetrical stripe down dorsum from shoulders to tail. CNS: Normal withdrawals, mildly prolonged paw replacement in hind limbs, R more prolonged than L. Assessment: Hindlimb lameness/neuro deficits/weakness - r/o neuro degenerative changes vs orthopedic vs other Low T4 - r/o hypothyroidism vs euthyroid sick Elevated SDMA/BUN, low USG - r/o early chronic kidney disease Stress leukogram, elevated ALP - r/o stress vs endocrine disease vs other Alopecia - r/o hypothyroidism vs other endocrine vs other Plan: Add on thyroid panel Trial gabapentin 300mg PO BID until otherwise directed for comfort CTM for decline in care
