Hello, my name is Sneakers. My animal id is #238646. I am a male brown cat at the Queens Animal Care Center. The shelter thinks I am about 7 years 1 weeks old.

I came into the shelter as a aco - impound on 10/6/2025, with the surrender reason stated as person circumstance- eviction.

Sneakers has been diagnosed with kidney disease and will need to follow up with a veterinarian for a long-term care plan. He is also underweight and has significant dental disease. He is very fearful in shelter and needs placement in a low stress environment.

DVM Intake Exam Estimated age: 6-8yo based on condition, dentition Microchip noted on Intake? scan neg History: From hoarding environment Subjective: BAR Observed Behavior - Flighty, attempted to flee and jump up on wall when let out, but able to capture with carrier. Able to examine and do procedures while covering patient with towel and applying light pressure to head and body Is there evidence of Cruelty? n Is there evidence of Neglect? n Is there evidence of Trauma? n Objective BCS 3/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Moderate staining, some calculus on caudal PMs, halitosis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, mildly increased BV sounds, eupnic ABD: Non painful, no masses palpated U/G: Male intact, 2 testicles palpable MSI: Ambulatory x 4, Moderate muscle wasting along epaxials. Recheck senior cat from multi-cat household, bloodwork results S/O: QAR at back of kennel resting in litterbox, good appetite, no c/s/v/d noted; suspect pu/pd - water bowl empty yesterday and today on rounds, appears to be large amount of urine in LB under cat EENT: No ocular or nasal discharge LUNGS: Eupneic CNS: Appropriate mentation CBC: Hct 22.1 (L), eosinophils 0.078 (L), basophils 0.14 (H) CHEM: SDMA 36 (H), BUN 64 (H), creatinine 1.6; sodium 169 (H), chloride 134 (H), albumin 2.3 (L), TT4: WNL A: Suspect pu/pd Anemia, non-regenerative vs pre-regenerative - ro chronic disease vs other Azotemia - ro pre-renal vs renal vs post-renal Hypernatremia, hyperchloridemia Hypoalbuminemia Periodontal disease Underweight P: Move to ICU for closer monitoring Collect urine for UA (ideally cysto) CTM closely while at QACC, recheck in 2 days

Recheck, UA results S/O: QAR at back of kennel, dilated pupils, pulls back when door opened; great appetite, no c/s/v/d noted, large amount of urine in LB EENT: No ocular or nasal discharge LUNGS: Eupneic CNS: Appropriate mentation UA (cysto): USG 1.019, pH 6.0, protein 3+, trace blood/hemoglobin A: Inappropriate urine concentration + azotemia - CKD Proteinuria - ro artifact (blood from collection method) vs true proteinuria PU/PD Anemia, non-regenerative vs pre-regenerative - ro chronic disease vs other Hypernatremia, hyperchloridemia Hypoalbuminemia Periodontal disease Underweight P: No treatment at this time - ideally SQ LRS administration, but patient has very high FAS Monitor closely on rounds and seek placement for long term care *Chronic kidney disease is a chronic, progressive disease in cats. Sneakers will need to see a vet and come up with a long-term plan for managment.

Recheck CKD cat S/O QAR, hides in back of the kennel when approached, wide eyes, hissed, no c/s/v/d appreciated, food trays disturbed, no ocular or nasal dc, eupneic, amb x 4 A. CKD High FAS P. Still not scheduling SQF d/t behavior and p is eating well CTM and recheck as scheduled

Brief recheck - QAR at back of kennel, good appetite, no c/s/v/d noted, no ocular or nasal discharge, eupneic. PLAN: CTM in ICU, seek placement for continued management of CKD (+ other problems)

BEHAVIOR SUMMARY:: Sneakers was lying in the back of his kennel as the assessor approached, body and face tense as he's hidden behind a large bed. Sneakers avoided eye contact when spoken to and he remained frozen in place as the kennel door opened. Sneakers tolerated petting along his head and body while flinching hard and wriggling around as the scratcher tool passed along his back. Pick up was not attempted at this time to reduce stress. Sneakers tolerates attention and petting but may be fearful or stressed in the shelter, and may be intimidated by small children. He may be a little more independent, and may need time to warm up to his new home. Due to the behaviors seen in the care center, we feel that this cat will do best in an experienced, adult only home.
