I came into the shelter as a owner surrender on 6/4/2026, with the surrender reason stated as animal health - will need ongoing vet care.

DVM Intake Exam Estimated age: Reported 4y, but difficult to assess age. 4-10y Microchip noted on Intake? scanned negative History: Owner surrender due to unable to provide medical care. O reports cat has always been unthrifty and has had a poor appetite. Initially was coming in for euthanasia request. Subjective: BAR Observed Behavior - Eating with very good appetite. Allows all handling for exam and all tasks. Evidence of Cruelty seen -no Objective T = P =wnl R =wnl BCS 2/9 EENT: Eyes pupils dilated OU unable to visualize any retinal vasculature, central opacities noted in lenses OU, ears clean, no nasal or ocular discharge noted Oral Exam: adult dentition with moderate tartar on upper premolars PLN: No enlargements noted H/L: NR, NMA, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: female intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - ataxia and intermittent tremoring noted, decreased menace response OU, palpebral intact Rectal: normal external Wood's Lamp Exam: not performed Assessment emaciation stunted growth ataxia-mild Prognosis: guarded Plan: intake tasks cbc/chem/T4 in house 0.25ml vitamin B12 1000mcg/ml sc once SURGERY: Permanent waiver due to poor condition

CBC/chem/T4 CBC HCT 29.4 % Chem SDMA 16 ug/dL (H) ALT 148 U/L (H) T4 WNL diagnosis is open consider BA stim test as next step needs placement for further workup

Recheck Exam BAR, eating very well, comes to front of kennel when approached No vomiting/diarrhea reported EENT: No ocular or nasal discharge, suspect cataracts OU H/L: Eupneic, normal RR CNS: Ataxic and tremoring intermittently, possible difficulty prehending food intermittently Assessment: Emaciated Stunted growth Ataxia and tremors Plan: CTM appetite, weight, GI and neuro signs Recommend EHR if not showing significant improvement in care, otherwise needs further workup with placement

BEHAVIOR DETERMINATION: : Level 1

BEHAVIOR SUMMARY:: Bebe Boop was sitting in her litter box with dilated eyes and upright ears. She approached the handler on shaky legs and eagerly ate the treats offered to her while accepting pets all along her head and back. She continued to allow all handling, remaining calm and tolerant during pick up. When placed down, her shaking continued but she returned to the front and leaned into pets curiously while she continued eating. Bebe Boop interacts with the assessor, solicits attention, is easy to handle and tolerates all petting. This cat is suitable for any level of adopter experience.
