DVM Intake Exam History: ACO impound, litter of 6; littermates have ringworm lesions (woods lamp positive) Estimated age: Approx 3 weeks Microchip noted on Intake? Negative Subjective: BAR, active, eating well, no c/s/v/d noted Observed behavior: Soft body, allows all handling Is there evidence of Cruelty - none Is there evidence of Neglect - none Is there evidence of Trauma - none Objective P = wnl R = wnl BCS = 5/9 EENT: Eyes - mild blepharospasm and third eyelid elevation OU, no nasal discharge noted, ears clean Oral Exam: mm pink and moist, CRT <2 PLN: No enlargements noted H/L: NMA, SSP; Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Female intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, areas of crusty fur with no notable alopecia CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Externally normal Wood's Lamp Exam: crusted regions on fur glow, but not apple green Assessment: Ringworm (littermates have classic lesions, woods lamp positive) Neonate, orphaned Conjunctivitis/URI Prognosis: Guarded Plan: Pooled sample for ringworm PCR Start lyme dips biweekly Start fortiflora PO SID to be shared amongst litter Start ofloxacin OU BID x 5 days Start itraconazole 10 mg/kg PO SID x 21 days Recheck weekly SURGERY: Temporary waiver due to age

[Spay/Neuter Waiver - Temporary] Your newly adopted pet has been diagnosed with being underweight and the staff veterinarians are issuing a TEMPORARY waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment through to the resolution of the issue. At the time of a full recovery you may choose to have your veterinarian perform the spay/neuter surgery, or make provisions to return the pet to ACC for sterilization.

Ringworm PCR positive for microsporum spp, microsporum canis. Continue with current treatment plan and recheck as scheduled.

recheck RW kittens- S/O BAR, A+A, vocalizing when approached, reportedly eating well, no c/s/v/d appreciated EENT: no ocular or nasal dc HL: eupneic MSI: amb x 4, woods lamp positive area the face, esp muzzle A. RW (under tx) P. P still falls within the dosage range for RW tx, con't with current treatments and recheck as scheduled. CTM while at QACC
