DVM Intake Exam Estimated age: Reported 11m Microchip noted on Intake? scanned negative History: Owner surrender. Reported progressive HL weakness. Reported worked up several times at outside vet hospital and cause of pain has remained a mystery. Subjective: BAR Observed Behavior -Wagging tail, but not wanting to stand up. Allows all handling for exam. Evidence of Cruelty seen -no Evidence of Neglect seen-no Evidence of Trauma seen -no Objective T = 103.6 P =wnl R =wnl BCS 5/9 EENT: Eyes clear, ears clean, no ocular discharge noted, clear nasal dc apparent Oral Exam: retained upper deciduous canine PLN: No enlargements noted H/L: NR, NMA, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: male intact, 2 scrotal testes MSI: Non ambulatory, swelling of carpal joints and around metatarsals all limbs, pain response on manipulation/palpation, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities, withdrawal reflex intact all 4 limbs Rectal: dried diarrhea on hind end Wood's Lamp Exam: not performed Assessment polyarthropathy-r/o Hypertrohic osteodystrophy (HOD) v infectious: Lyme, ehrlichia, coccidioides v septic arthritis v other pyrexia Prognosis: guarded Plan: sedated for radiographs using 0.2ml dexmedetomidine 500mcg/ml and 0.2ml butorphanol 10mg/ml IM distal physes of radius bright white line, on radius indistinct and expansive lesion, similar expansive lesion distal tibia in house CBC/chem blood to lab for 4dx with coccidioides AGID 100ml LRS sc SID x5d 0.35ml carprofen sc once-consider extending if helpful 0.3m buprenorphine 0.3mg/ml IM BID x5d Recommend placement ASAP for joint tap for cytology SURGERY: Temporary waiver due to poor condition
