No Objective: P = WNL R = WNL BCS 4/9 EENT: OS WNL, OD mild blepharospasm with mild mucoid ocular discharge at medial canthus, ears clean, no nasal or ocular discharge noted Direct and consensual PLRs intact Menace and palpebral positive OU Oral Exam: Clean dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2s, Lungs clear, eupneic, negative ITC ABD: Soft, nonpainful, no masses palpated U/G: Male - intact, two symmetrical scrotal testicles MSI: Ambulatory x 4 - see CNS, full orthopedic exam not performed INT: Healthy hair coat, no masses noted, no ectoparasites seen CNS: Mentation appropriate, severe ataxia x4 with wide based stance and hypermetric gait. Withdraw positive x4. Able to ambulate, falls over occasionally, no rolling. Seeks out wall for stabilization. [Spay/Neuter Waiver - Permanent] Your newly adopted pet has been diagnosed with NEUROLOGICAL ABNORMALITIES and the staff veterinarians are issuing a PERMANENT waiver from the spay/neuter requirements of the City of NY. ACC does not have the facility to safely sterilize your pet with his/her current condition. However, your veterinarian will provide consultation on whether this procedure can or should be performed under their supervision. Follow up care at your regular veterinarian is recommended to ensure continued treatment and proper oversight of your pet's health. All costs for follow up care and subsequent surgery is at the expense of the adopter.

recheck, suspected severe CH cat S/O BAR, A+A, solicits attn, comes to the front of kennel for interaction, observed pecking at dry food and able to crouch down to drink water, noted to be urinating/defecating, LB just cleaned EENT: mild serous dc OU, no nasal dc, mm pm <2 HL: eupneic MSI: amb x 4 although with difficult d/t balance issues, good skin tent CNS: ataxic, wide based stance but able to peck at food to eat and use stance to drink water, ambulatory but falling to either side intermittently, able to get up without assistance, intention tremors A. Suspected CH- moderate/severe, able to eat/drink with little help and urinate/defecate, unclear if in litter box Conj- improved from intake exam P. CTM in medical- neuro signs appear similar to those on presentation for intake, suspected CH, CTM for change in neuro status but suspected baseline Recheck in 2 days seek placement
