DVM Intake Exam Estimated age: 11 years Microchip noted on Intake? Scanned negative History: Owner surrender Subjective: BAR Observed Behavior - Soft body for exam. Rolls over for belly rubs. Is there evidence of Cruelty? No Is there evidence of Neglect? No Is there evidence of Trauma? No Objective P = 120 R = WNL BCS 5/9 EENT: AU: cropped pinna, narrowed canals with brown discharge. Eyes clear. No nasal or ocular discharge noted. Oral Exam: Blackened tartar and severe gingivitis PLN: No enlargements noted H/L: NR, NMA, Lungs clear, eupneic ABD: Soft, non-painful. No masses palpated U/G: Female presume intact despite large ventral midline scar. 3 cm soft swelling ventral to left 4th nipple. Pendulous mammary glands. MSI: Approx 1.5 cm reducible umbilical hernia. Hernia is not sensitive on palpation. S: QAR, tachypneic + coughing Pulse ox: 95% O: EEN- nuclear sclerosis OU, green nasal discharge, dried crusted nasal planum H/L- Tachypnea, harsh lung sounds MSK/i- Overweight, dry haircoat Neuro- alert/appropriate A: CIRDC +/- pneumonia Anorexia P: Move to med ISO Place IV catheter (awaiting t-ports since out of stock) Begin fluids @ 75ml/hr Cerenia 10mg/ml IV 2.0 mls q24hr x5d Enrofloxacin 100mg/ml 2.0 ml IV diluted to 4 mls in sterile water and given over 10 minutes q24hr x5d Monitor

Hx: lethargy; coughing; not eating well QAR; H pink eent- no mucoid nasal discharge; coughing episodes hl- coughing; 120 hr msi- amb x 4 A) r/o pneumonia vs other P) Continue enrofloxacin- 100mg/ml (10 mg/kg IV SQ PO SID) (IM causes tissue necrosis- can be given once) sig: 2 1/2 ml SQ IV DILUTE (to prevent necrosis) IV fluids- LRS 47 ml/hr Entyce 30mg/ml 1.7 ml po sid x 3days (started)

Prognosis: Poor Plan Removed IVC LRS 300 mls SQ Enrofloxacin 100mg/ml 1.6 mls diluted to 4 mls with sterile water sid x 5 days Clindamycin 150mg + 75 mg 1 tablet of each bid x 7d

How is this dog around cats?: Will snap at cats.

For a New Family to Know: Chi Chi is a quiet and independent dog. She spends most of her time in the living with or with people. She lets you know when someone is at the door with a light bark. She's very food motivated and loves all types of treats. She's a good walker, will walk by your side or behind you. Loves going on car rides, it's literally her favorite thing in the whole entire world. You need to bribe her with a treat to get out of the car!

Summary (7):: 11/6/24: Chi Chi is at the front of her pop up crate with a soft body and wagging tail. She is easily leashed and removed from kennel. In the hallway, she will walk up to staff, loose and wiggly body, looking straight up at them. If petted, she will sit and lean in. She is walked on the street where she walks slowly, sniffing. When another dog passes by, she is loose and wiggly pulling forward. In the behavior room, hi Chi approaches handlers with a loose and wiggly body looking up at them. She sits at handlers feet and leans in for pets. She is easily collared and returned to kennel.
