I came into the shelter as a aco - impound on 9/14/2025.

Buddy has been added to the At Risk List for medical reasons. Buddy is one kitten in a litter of 6 that have been diagnosed with ringworm and various degrees of URI/conjunctivitis. These kittens need placement outside of the shelter for appropriate treatment and follow up of their medical concerns.

DVM Intake Exam History: ACO impound, litter of 6; littermates have ringworm lesions (woods lamp positve) Estimated age: Approx 3 weeks Microchip noted on Intake? Negative Subjective: BAR, active, eating well, no c/s/v/d noted Observed behavior: Soft body, allows all handling Is there evidence of Cruelty - none Is there evidence of Neglect - none Is there evidence of Trauma - none Objective P = wnl R = wnl BCS = 5/9 EENT: Eyes - blepharospasm and mucoid discharge OU, crusted/dried nasal discharge noted, ears clean Oral Exam: mm pink and moist, CRT <2 PLN: No enlargements noted H/L: NMA, SSP; Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Male intact, two testicles palpated MSI: Ambulatory x 4, skin free of parasites, no masses noted, crusting along forelimbs and face - no obvious areas of alopecia CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Externally normal Wood's Lamp Exam: crusted regions on fur glow, but not apple green Assessment: Ringworm (littermates have classic lesions, woods lamp positive) Neonate, orphaned Conjunctivitis/URI Prognosis: Guarded Plan: Pooled sample for ringworm PCR Start lyme dips biweekly Start fortiflora PO SID to be shared amongst litter Start ofloxacin OU BID x 5 days Start itraconazole 10 mg/kg PO SID x 21 days Recheck weekly SURGERY: Temporary waiver due to age

[Spay/Neuter Waiver - Temporary] Your newly adopted pet has been diagnosed with being underweight and the staff veterinarians are issuing a TEMPORARY waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment through to the resolution of the issue. At the time of a full recovery you may choose to have your veterinarian perform the spay/neuter surgery, or make provisions to return the pet to ACC for sterilization.

Staff noted that p were receiving gruel and unable to mix in the forti flora. Told staff to start offering gruel, d/c fortiflora.

recheck eyes, last day of oflox S/O BAR, A+A, vocalizing for food, no c/s/v/d appreciated or noted, noted to be eating well. All kittens in this litter still have mucoid ocular dc OU to varying degrees. When eyes cleaned, mild conjunctival hyperemia and chemosis. All kittens in litter appear to have some degree of microphthalmia A. conjunctivitis r/o second to microphthalmia, other congenital ocular abn, open P. Extend oflox for 3 days Con't with current treatments and recheck in 3 days
