[Spay/Neuter Waiver - Upper Respiratory Illness] Your newly adopted animal is in treatment for an upper respiratory illness. The veterinarian is temporarily waiving this animal from the spay/neuter requirements of the City of NY until such time as the illness has resolved and the pet has sufficiently recovered. At that time, this animal must come into compliance with the spay/neuter requirements.

Progress exam History :stray intake 2/16. Started on terramycin, nebulization, and given ivermectin. Subjective: QAR. Food untouched. Congested with nasal and ocular d/c. No csvd. Objective EENT: serous ocular d/c and conjunctivitis, congestion w/ mild serous nasal d/c H/L: Eupneic, normal RR/RE, congested MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Assessment: URI with conjunctivitis Gingivitis Ear mites on intake Prognosis: Good Plan: Iso Start doxycycline 10mg/kg PO SID x10d until 2/28 (first dose given) Continue terramycin until 2/22 Continue nebulization until 2/22 Recheck day 7 Iso Recheck appetite tomorrow-offer variety of food today

Tongue sticking out likely from missing central incisors (noted on intake). Has eaten several types of variety food offered.
