[Spay/Neuter Waiver - Temporary] Your newly adopted pet has been diagnosed with RINGWORM and the staff veterinarians are issuing a TEMPORARY waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment through to the resolution of the issue. At the time of a full recovery you may choose to have your veterinarian perform the spay/neuter surgery, or make provisions to return the pet to ACC for sterilization.

Recheck, ringworm PCR positive S/O: BAR, active, good appetite, no c/s/v/d noted, no ocular or nasal discharge, eupneic. Ringworm PCR - positive for microsporum spp. + microsporum canis A/P: Ringworm; continue itraconazole SID and biweekly lyme dips. Recheck as scheduled.

Dermatophytosis PCR panel: Microsporum spp. RealPCR --POSITIVE Microsporum canis RealPCR -- NEGATIVE Trichophyton spp. RealPCR -- NEGATIVE Progress exam S/O: Weight today is 2.91 lbs BAR, very friendly and social, leaning into head rubs and pets. Alopecia on lateral aspect of both ears, no other obvious areas of hair loss or crusts. Wood lamp: no fluorescence on areas of alopecia and previously noted lesions. A: Positive for microsporum spp - r/o true vs false positive Gaining weight P: Adjust itraconazole dose based on current weight (2.91 lbs), 7.5mg/kg = 1ml Extend treatment for another 3 weeks (itraconazole + lyme sulfur twice weekly) Repeat PCR 1 week from previous test (day 28) *DVM to collect PCR

Coat wiped with dampened paper towel. Ears, tail, and hind toes brushed with toothbrush, then hairs plucked from both ears, hind toes, and tail.

Ringworm week recheck Subjective: BAR, no C/S/V/D. AS:2 ate medications (poked head out of den but did not fully come out) Objective: EYES: OS Squinting. Mild clear mucoid discharge in eye, crusted dark brown discharge around eye. OD Clear LUNGS: Eupneic, no sign of respiratory distress NASAL CAVITY: no discharge MUSCULOSKELETAL: Ambulatory x4, no lameness or obvious ringworm lesions noted cage side NEURO: Appropriate mentation ASSESSMENT: Ringworm appears stable PLAN: -Continue current treatment plan, DVM recheck

Recheck ringworm, PCR remains positive S/O: BAR, playful, eating well, no c/s/v/d noted MSI: Amb x 4; pinnae are fully alopecic with shiny/smooth skin and no evidence of crusting or fur regrowth, focal alopecia at dorsal head with very faint scaling Ringworm PCR - positive for microsporum spp A: Dermatophytosis P: Continue treatment itraconazole PO SID and biweekly lyme dips Seek placement for more controlled treatment and individualized care - ringworm typically clears by now with appropriate treatment CTM while at QACC
