DVM Intake Exam Estimated age: 5 years Microchip noted on Intake? Scanned positive History: Owner surrender due to persistent tail chasing since dog was young. Worsened when his canine house mate was rehomed. Owner reports tail chasing is worse out of the house than in the house. Subjective: QAR Observed Behavior - Hard barking while wagging tail. Allowed leashing but then started charging kennel door. Elected to sedate for exam. Is there evidence of Cruelty? No Is there evidence of Neglect? No Is there evidence of Trauma? No Objective P = 120 R = WNL BCS 5/9 EENT: Eyes clear. Ears clean. No nasal or ocular discharge noted Oral Exam: Moderate tartar. PLN: No enlargements noted H/L: NR, NMA, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Male neutered. Green tattoo present. MSI: Thickened dry skin with hemorrhagic crust and thinned hair at tail tip. Epidermal collarettes ventral chest. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: WNL externally Wood's Lamp: Negative Assessment Chronic tail chasing r/o anxiety vs other Dermatitis tail tip Pyoderma ventral chest Prognosis: Poor Plan: Intake tasks chem cbc hwt E-collar indefinitely Trazodone 100mg 1 tablet po bid indefinitely (5.2 mg/kg bid) Chlorhexidine mousse on tail tip and ventral chest 3 x week for 2 weeks. +/- Fluoxetine 20mg 1 tablet po sid x 4 to 8 weeks then reassess (1mg/kg). Advise Royal Canin Hydrolyzed protein or Purina Hypoallergenic or Science diet z/d diet trial x 2 months. Advise placement SURGERY: Already neutered

Hx: noted during rounds lethargy; labored breathing; moving very slowly; has been placed on behavior modifying medications secondary to tail chasing QAR; H-DH pink; eating food offered eent- mucoid nasal discharge; coughing on presentation hl- increased abdominal component on inspiration; respiratory sinus arrhythmia; no obvious wheezes or crackles msi- amb x 4 but slowly A) slow gait r/o secondary to behavior medications increased abdominal component r/o pulmonary disease vs other P) Dog sedation: Using 0.7 ml dexmedetomidine at 500mcg/m2 (500mcg/ml) and 0.7 ml butorphanol at 0.4 mg/kg (10mg/ml) IM Reversed with Antisedan-0.7 ml IM CHEST RADS: alveolar pattern r/o pneumonia Enrofloxacin- 10 mg PO SID sig: 204 mg po q 24 hrs x 10 d's SQ fluids- LRS 500 ml q 24 hrs

EEN: No ocular or nasal discharge RESP: Increased respiratory effort MSI: Ambul x 4 Sways in hind end while standing to eat. NEURO: QAR A: Excess sedation. Good appetite CIRDC P: Decrease gabapentin to 300mg po bid Removed e-collar

upon intake, Blue had a neutral body pose and socially jumped on me when owner greeted me outside the MACC. Blue body became tense once inside the lobby. didn't collar due to influx of intakes. allowed scanning. mc positive. allowed me to place slip leash with ease. walked along side me towards hallway door and then froze and became very tense. Owner came over to help Blue come inside the hallway. Once he was inside the hallway and making our way towards intake room, Blue froze and began to pancake a bit. Staff who was next to me, helped me by using his leash as a belly band to help me take blue towards the intake room as he was very hesitant to walk.
