DVM Intake Exam Estimated age: ~14wo MI DSH Microchip noted on Intake? Scan neg History: Brought in as stray by caretaker Subjective: BARH, no food eaten overnight Observed Behavior - Meowing in carrier. Attempts to flee when handling but easily restrained. Able to examine with mild restraint. Is there evidence of Cruelty? No Is there evidence of Neglect? No Is there evidence of Trauma? No Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Two sets of adult incisors present, deciduous canines. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Male intact, 2 testicles MSI: Ambulatory x 4, abnormal RH foot but able to bear weight on medial aspect of foot. No crepitus palpable. Skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Ext normal Wood's Lamp Exam: NP Assessment ~14wo MI DSH Multiple metatarsal fractures (MT3, MT4) with displacement of digits, deformed foot as a consequence Ambulating on digit 2 Prognosis: Fair Plan: Routine intake procedures Sedating for neuter and RH foot rads with: Torb 0.055ml, dex 0.055ml, ket 0.11ml IM Simbadol 0.3ml SQ Neutered - see neuter report Distal RH limb 2 view: MT 4 fracture and displacement of distal segment; MT 3 SH5 fracture with displacement of P3. No boney calluses noted, no obvious evidence of healing. Recheck tomorrow and give additional pain control if needed. Recommend consult with ortho specialist to assess for possible surgical repair vs digit amputation vs whole leg amputation - concerned about long term consequences of non-healing fracture and foot deformity SURGERY: Neutered

recheck kitten with metatarsal fx- S/O QAR, hiding in den, food appears eaten, no c/s/v/d appreciated or noted, no ocular or nasal dc, eupneic. *when staff reached in to get p, possible touched the RHL with fx, p hissed and tried to flee. Elected to apply pain meds and put in recheck to reassess. A. Fx RH metatarsal P. Applied zorbium pink Seek placement- p is kitten with fractures, one SH fx, concern for how the bone will heal long term Recheck in 1-2 days when pain better under control CTM closely on rounds

Progress exam S/O: Patient is resting in den. Retreats when attempting to greet. After spending some time quietly, gave slow blinks and relaxed in den. Able to ambulate in kennel, appetite score noted as 2. A. R metatarsal fractures, nonhealing but ambulatory P. Zorbium applied 12/21, due 12/24 Continue to monitor - recommend sending rads for ortho consult with A vs transfer ASAP for better assessment

recheck- RL metatarsal fx, received zorbium last 12/25 (NOTE: appears to show received 12/24 in SB when log book notes it on 12/25- confirmed with staff received 12/25) S/O QAR, retreats into kennel, brought out onto exam table, no c/s/v/d appreciated or noted, food trays moderately disturbed EENT: no ocular or nasal dc HL: eupneic MSI: amb x 4, p very skittish but appears to be bearing weight on the RHL A. Fracture metatarsals RHL P. Okay to move out of ICU Due for next zorbium 12/28 (not 12/27) Repeat rads in 4-6 weeks. Ideally patient would be splinted but as already being confined to kennel/movement restricted. Recheck in 3 days An adopter should be aware that p would likely require splinting and 4-6 weeks of minimal activity/cage rest as leg heals

Fractured metatarsals High FAS P. Difficult to assess if patient is comfortable d/t to high FAS- doesn't appear to req Start gaba 10mg/kg PO BID x 7 days recheck in 2 days CTM while at QACC

Recommendations comments:: No young children under 13 due to fearfulness. Older respectful children only. Counselor discretion.
BEHAVIOR SUMMARY:: Upon approach, Sol was inside his pressed up against the back with wide eyes, forward ears, tense body and face, and tail wrapped tightly around his body. As the kennel door was opened, the assessor offered Sol the scratcher. He leaned in to sniff but then quickly backed away and tucked himself into the corner. Reaching the scratcher further into the den, the assessor was able to give Sol a few body pets. He remained tense with wide eyes and began to low growl. Pick up was not attempted at this time. Sol tolerates attention and petting but appears fearful or stressed in the shelter. He may be a little more independent, and may need time to warm up to his new home. We recommend this cat go to a home with experienced cat parents. Due to the behavior shown, he may not be a great fit for young children. Any home with older children prepared to adopt this cat should conduct a thorough interaction and endeavor to monitor this cat around their children to prevent these behaviors from being reinforced.
