No Objective: T = P = wnl R = wnl BCS 3/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Moderate dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: Externally normal intact male, two descended testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy, dirty hair coat CNS: Mentation appropriate - no signs of neurologic abnormalities Rectal: Rectum empty, normal prostate (symmetrical, not enlarged), anal glands full, feces bloody and liquid, no abnormalities palpated Wood's Lamp Exam: Not performed Assessment: Melena r/o parasites vs GI ulcer vs toxicity vs IBD vs pancreatitis vs neoplasia Prognosis: Fair Plan: Intake exam and tasks Sedated radiographs - 0.4ml (0.5mg/ml) dexdorm, 0.8ml (10mg/ml) torb - Gas filled intestines, not dilated, stomach mildly distended with what appears to be food - suspect enteropathy Repeat parvo test - negative CBC/Chem/T4 in house - Mild leukocytosis and neutrophilia, otherwise no significant findings Start Fortiflora PO SID for 7 days Start panacur 5g PO SID for 5 days Start GI diet for 7 days Recheck in 5 days SURGERY: Temporary waiver due to illness

Continued melena reported on rounds. Patient is BAR and energetic. Placed monitoring sheet to evaluate appetite and any other clinical signs. Already on supportive care. - Add Sucralfate 1g tablet BID x 5 days Continue to monitor.

S: Recheck Melena Noted AS 1-3 (AS 2 today). Continued diarrhea with FS6 + blood noted today. BAR, BCS 3/9. Recheck weight 46.5lbs (stable since intake). P: Extend sucralfate 1g tablets PO BID x additional 5 days Add famotidine 20mg BID x 5 day Extend GI diet, add third feeding. OK to feed treats/chicken to tempt food. Recheck scheduled x 5 days

POCUS: No free fluid in abdomen Unable to evaluate stomach due to ingesta Intestines: no obvious masses Spleen hypoechoic to liver Gallbladder: moderate size with hypoechoic contents Kidneys not remarkable Bladder small with hypoechoic contents No pericardial effusion present A: r/o occult gastric ulceration/neoplasia/foreign body +/- extramedullary hematopoiesis vs severe IBD vs other neoplasia Prognosis: Guarded to poor Plan Advise placement for abdominal ultrasound and endoscopy with internist +/- blood transfusion.
